• After the minimum period, the account will continue to be charged at the agreed monthly rate unless I give Body by Me Health Club written notification to temporarily suspend or cancel my membership.
• Notification to temporarily suspend or cancel my membership must be in writing and within 30 days of the next billing date to be successful. If notification of intent to suspend or cancel is not received 30 days prior, the account will be charged as agreed.
• The above membership rate will apply unless the club provides me with a minimum 30 days written notice of fee changes.
• A $150 cancellation fee will apply should I choose to cancel my membership prior to the minimum membership period of 12 months.
• I am entitled to a maximum of 4 weeks suspension time with a minimum amount of time being one week
• Membership suspension must be applied for in advance and will not be backdated under any circumstances
• Timetables, opening hours and facilities may be altered by management to suit seasonal and usage requirements.

I, and if being a minor, my parents/guardians, for and on behalf of myself, acknowledge that during all such times as I am on the premises of or included in any activity external to the premises which is organized, approved or endorsed by Body by Me as an activity for me to take part in, both my property and person shall be at me own risk and I will not hold Body by Me liable for any personal injury or loss of property which may arise from the negligence of Body by Me, its servants, agents, independent contracts, voluntary workers, other users of the facility or participants in the activities or spectators or other parties providing services through or in the facilities of Body by Me. I warrant that I am physically fit and able to engage in exercise and fitness programs with Body by Me.
